About our project and product
Our client, Simone Thannhauser, envisioned a more connected and transparent way for families and support workers to celebrate participants’ progress.
To bring this vision to life, our team developed CORE Transitions — a secure, easy-to-use web platform that tracks individual goals, milestones, and activities while encouraging collaboration between participants, guardians, and support workers. Designed for accessibility and clarity, the system turns goal tracking into a shared experience, empowering participants to visualise their achievements and stay motivated on their journey.
